ABSTRACT

Objective To investigate the diagnostic value of random forest(RF)model based on CT images and clinical data for preoperative T staging of colorectal cancer. Methods Four hundred and fifty patients with colorectal cancer who were pathologically confirmed by surgery and underwent preoperative CT examinationinthe first hospital of Jilin university from January 2016 to July 2016 were included retrospectively(Stage≤T2,T3,and T4 each has 150 cases).According to the ratio of 2:1,the patients were divided into training set(300 cases)and test set(150 cases, stage ≤T2,T3,and T4 each has 50 cases)by computer random software. Each of 450 patients had one lesion. All the patients underwent preoperative abdominal and pelvic contrast-enhanced CT scan. The clinical, imaging and pathological data[gender, age, carcinoembryonic antigen (CEA) level, carbohydrate antigen 19-9 (CA19-9) expression, intestinal wall deformation, maximum diameter of tumorand thickness of intestinal wall, location, enhancement homogeneity and enhancement rate]of these patients were collected. The correlation between the collected factors and pathological T staging was analyzed by Spearman correlation analysis. The preoperative staging model of colorectal cancer was established by RF algorithm in the training set.Two kinds of methods(model and traditional method)were used to diagnose T stage of the patients in the test set.The accuracy of the two methods was calculated by postoperative pathological staging as the gold standard. The consistency test was used to evaluate the consistency of the RF model results with the pathological results. Results T-staging was positively correlated with CEA, CA19-9, intestinal wall deformation, tumor size and thickness of intestinal wall(r=0.449,0.291,0.624,0.573,0.386;P<0.05).Age,location,enhancement homogeneity and enhancement rate were slightly negatively correlated with T-staging(r=-0.115,-0.245,-0.120 and-0.339;P<0.05).The predictive results of the model in≤T2,T3,and T4 stage cancers were moderately and highly consistent with the standard of pathology, and the Kappa value were 0.769,0.615 and 0.800,respectively.The total accuracy rate of the model andthe traditional method are 80.7%(121/150)and 54.0%(81/150). Conclusion Application of random forest model based on multi-slice spiral CT images and clinical data can improve the diagnostic efficacy of preoperative T stage of colorectal cancer.

ABSTRACT

Aim The TRPV1 plasmid was transiently transfected into human embryonic kidney HEK 293T cells to establish the heterologous expression system of TRPV1-channel. Methods The transfection efficiency was confirmed under fluorescence mi-croscope and the TRPV1 protein expression was identified by u-sing Western blot, and the functional characteristics of the chan-nel were studied by using the method of confocal microscopy. Results The transfection rate could reach 40% ~50%; the transfected cells were found to have a clear band at the corre-sponding position that TRPV1 should be, which indicated that TRPV1 channel protein was expressed in the transfected cells. The confocal microscopy imaging result showed that the trans-fected HEK 293T cells were activated by TRPV1 channel ago-nist. Conclusion Transient transfection of HEK 293T cells with TRPV1 channel is successfully constructed and the heterol-ogous TRPV1 channel is verified to have normal calcium-media-ting function.

ABSTRACT

Aim To further study the molecular mecha-nism of the herbs with hot nature on the regulational action on TRPV1 channel based on the 7900 Real-time PCR instrument. Methods 7900 PCR instrument was applied to detect the intracellular flurescence of TRPV1 channel in the dorsal root ganglions ( DRG ) neurons and the effect on the TRPV1 ’ s thermo-sensational functions of the selected 11 ingredients from hot herbs was explored. Results TRPV1 channels could be ac-tivated by gradually elevated temperature; the activa-tion process could be blocked by the TRPV1 specific blocking agent capsaizepine. Most of the ingredients from hot-nature herbs had the potential to up-regualate TRPV1 channel function. Conclusions The estab-lished TRPV1 channel detection system based on PCR instrument is suitable for the analysis of regulational functions of drugs on the heat-activated TRPV1 chan-nel;the functions of hot herbs may be related to the up-regualtional effects of its active ingredients on the TRPV1 channel which will further up-regulate energy metabolism.

ABSTRACT

Purpose Two kinds of forward conditional reasoning including modus ponens (MP) and denial of the antecedent (DA) are the most common deductive reasoning in daily life. The present study aimed to explore the difference between the two neural machanisms. Materials and Methods Three kinds of tasks including MP, DA, and baseline were administrated to 14 undergraduates [7 males and 7 females, mean age (23.4±1.3) years] by using functional magnetic resonance imaging (fMRI) technique. fMRI data was analyzed. Results The reaction times were significantly slower for MP and DA than that of BS (P<0.001), while the effect was not significant for accuracy. In contrast to baseline, both MP and DA tasks activated the left inferior frontal cortex (BA 9), inferior parietal lobule (BA 40), and postcentral gyrus. Bilateral caudate significant activation was found in MP task when compared with DA, whereas the frontal, parietal, occipital lobe and anterior cingulate cortex were acivated more in the DA than in MP task. Conclusion Both forward conditional reasoning of MP and DA commonly activate the left frontoparietal cortex, while significant dissociations can be seen in the regions of caudate and bilateral frontal, parietal and occipital. These results firstly demonstrates the different neural mechanism underlying the two forward conditional reasoning, which might help to further uncover the brain mechanism of conditional reasoning.

ABSTRACT

Objective To detect the relationship between the molecular defects and their phenotypes in children with growth hormone insensitivity syndrome (GHIS).Methods 21 patients defined as GHIS were enrolled in the study.4 candidate genes (GHR,IGFALS,JAK2,and STAT5B) were analyzed by genomic DNA sequence screening and clinical relevance analysis. Results The statistical descriptions of the patients were showed as an average height standard deviation (SDS)-4.33 ± 1.91 (-9.17 to-2.21),average serum peak values of GH (22.67 ±20.98) tg/L (11.33 to 104.21 μg/L),basal serum insulin-like growth factor-Ⅰ SDS-2.65 ± 0.53 (-3.57 to -1.79),insulin-like growth factor-binding protein 3 SDS-1.77 ± 1.64 (-4.13 to 0.96).Bone age of backward difference (chronological age-bone age) (43.10 ± 19.54) months (6 to 82 months). One of two children with severe growth failure and mid-face hypoplasia was found to a homozygote for G to A gene mutation in the intron 6 splice donor consensus sequences (IVS6 ds+ 1 G-A) in the GHR gene, causing its functional defect.3 cases with mild dwarf were found gene variations as novel finding:c.1097T>C c.1098C>T p.V366A pathogenic variant,c.1229C>T p.S410L and nt1843707 A→G of 5' UTR region in the IGFALS gene.JAK2 and STAT5b genes mutations were not found. Conclusion Molecular pathology of GHIS is considered as involving the defects of GHR and its signal pathway. The mutation of intron 6 splice donor sequences in GHR gene has been reported which affect the function of GHR.The 3 novel type base variants in IGFALS gene, causing non severe dwarfism, might be suspected with pathogenic roles of GHIS.

ABSTRACT

BACKGROUND:A large number of clinical trials have found that the number of bone marrow stem cels at the femoral neck and proximal femur in patients with osteonecrosis of the femoral head is significantly reduced, accompanied by decreased activity, which causes a significant decrease in osteogenic capacity that the necrotic bone cannot be effectively repaired after absorption, leading to the colapse of the femoral head. OBJECTIVE:To probe into the early clinical efficacy of autologous peripheral blood mononuclear cel transplantation with porous core decompression for treatment of avascular necrosis of the femoral head. METHODS:Forty-five patients with avascular necrosis of the femoral head (49 hips) were enroled in this study, and underwent autologous peripheral blood mononuclear cel transplantation with porous core decompression. After treatment, pain scores, Harris hip score, scores on the satisfaction of patients were evaluated, as wel as X-ray, CT and MRI examinations. RESULTS AND CONCLUSION:Al the patients received a folow-up visit of 11-14 months, averagely (12.5±0.6) months. During the folow-up, there were no complications and serious adverse reactions. Postoperative pain scores and Harris scores were both improved significantly compared with preoperative ones (P < 0.05). At 12 months after treatment, the excelent satisfaction rate was up to 92%. Patient’s MRI low signal region accounting for a percentage of the volume of the femoral head was decreased from (40.1±7.34)% preoperatively to (20.23±5.4)% at 6 months postoperatively, and there was a significant difference (P < 0.05). These findings indicate that autologous peripheral blood mononuclear cel transplantation with porous core decompression for treatment of avascular necrosis of the femoral head has significantly clinical effects at early stage, which can obviously reduce joint pain, improve and restore hip joint function, and delay progression of disease.

ABSTRACT

BACKGROUND/AIMS: Patients with irritable bowel syndrome (IBS) are characterized by abnormal central processing with altered brain activation in response to visceral nociceptive signals. The effect of electroacupuncture (EA) on IBS patients is unclear. The study is set to study the effect of EA on brain activation during noxious rectal distension in IBS patients using a randomized sham-controlled model. METHODS: Thirty IBS-diarrhea patients were randomized to true electroacupuncture or sham acupuncture. Functional MRI was performed to evaluate cerebral activation at the following time points: (1) baseline when there was rectal distension only, (2) rectal distension during application of EA, (3) rectal distension after cessation of EA and (4) EA alone with no rectal distension. Group comparison was made under each condition using SPM5 program. RESULTS: Rectal distension induced significant activation of the anterior cingulated cortex, prefrontal cortex, thalamus, temporal regions and cerebellum at baseline. During and immediately after EA, increased cerebral activation from baseline was observed in the anterior cingulated cortex, bilateral prefrontal cortex, thalamus, temporal regions and right insula in both groups. However, true electroacupuncture led to significantly higher activation at right insula, as well as pulvinar and medial nucleus of the thalamus when compared to sham acupuncture. CONCLUSIONS: We postulate that acupuncture might have the potential effect of pain modulation in IBS by 2 actions: (1) modulation of serotonin pathway at insula and (2) modulation of mood and affection in higher cortical center via ascending pathway at the pulvinar and medial nucleus of the thalamus.

ABSTRACT

The hydroxypropyl methylcellulose (HPMC) matrix tablet containing metoprolol succinate (MS) as a model drug was obtained by wet method compression tablet. The effects of the amount and viscosity of HPMC, the preparation method, compressing pressure, the amount of ethycellulose (EC), the pH of dissolution medium and the speed of basket rotation on the drug release from the matrix tablets were evaluated. The results showed that the release rate of metoprolol succinate from HPMC matrix tablets followed Higuchi equation. The release mechanism was in line with the synthetical effect of diffusion and corrosion. Drug release was influenced by the amount and viscosity of HPMC, the amount of EC in matrices, the preparation method and compressing pressure, etc. Drug release was not influenced by the pH of dissolution medium used, and not by the speed of basket rotation, either.
